We take time to understand your lifestyle, routines, and goals, helping you explore the right SIL, STA, or MTA options and matching you with a consistent, qualified team you can trust. Our support workers provide respectful, reliable care while helping you build skills, maintain routines, and feel comfortable and connected. Working closely with your support network and NDIS partners, we ensure your care is seamless and tailored to your needs — creating a stable, supportive environment where you can settle in confidently and work toward long term independence.

Our Model of Care

Our hospital discharge support provides a safe, smooth, and stress free transition back home. We coordinate everything you need — from transport and medication support to home preparation and follow up appointments — working closely with hospital teams, therapists, and your support network. Our trained staff help you settle in comfortably, reduce risks, and regain independence with the right supports in place from day one. Guided by our person centred, strengths based Model of Care, we ensure nothing is missed and create a recovery focused plan that helps you continue healing confidently with all supports organised around you.

Services provided

Short Term Accommodation (STA) – Short stays with 24/7 support

STA provides respite or temporary stays in a supportive environment. It’s ideal for taking a break, trying new experiences, or having extra help when needed.

Supports include:
  • 24/7 care and supervision.
  • Personal care and daily living assistance.
  • Engaging activities, outings, and community participation.
  • Meal preparation and accommodation services.
  • Routine support and skill building.
  • Transport for activities or appointments.
  • Respite for families and carers.

Medium Term Accommodation (MTA) – Temporary housing while waiting for long term options

MTA offers stable, temporary housing with the right level of support while you transition to permanent accommodation, such as SDA or home modifications.

Supports include:
  • Safe, stable housing during transitions.
  • Assistance with personal care and daily routines.
  • Household tasks and meal preparation.
  • Support to build independence while waiting for long-term solutions.
  • Coordination with therapists, NDIS planners, and housing providers.
  • Emotional and practical support to reduce stress during the transition.

Supported Independent Living (SIL) – Ongoing daily support in your home

SIL helps you with everyday tasks, personal care, and building independent living skills so you can feel comfortable, safe, and confident at home.

Supports include:
  • Personal care and hygiene
  • Meal preparation and household tasks
  • Medication support
  • Community access and social participation
  • Daily routines and independent living skill development
  • Emotional and behavior support
  • Overnight support (sleepover or active nights)

Independent Living Options (ILO) - Flexible living support designed around how you choose to live

Independent Living Options (ILO) support people to design a living arrangement that reflects how, where, and with whom they want to live, while receiving the right level of support to live safely, confidently, and independently.

Supports include:
  • Assistance to design and implement personalised living arrangements aligned with individual goals
  • Support to build daily living skills and independence within the home
  • Emotional regulation and wellbeing support to maintain stability and confidence
  • Development of routines and structures that support daily functioning
  • Support with decision making, problem solving, and self management skills
  • Guidance to build informal supports and positive relationships within the home environment
  • Strategies to promote safety, wellbeing, and long term sustainability of the living arrangement
  • Ongoing mentoring and support to adapt arrangements as needs or circumstances change

Specialist Disability Accommodation (SDA) - Purpose built housing support for people with high support needs

Specialist Disability Accommodation (SDA) provides specially designed housing that improves safety, accessibility, and independence for people with significant functional or psychosocial support needs.

Supports include:
    • Support to live in SDA approved homes suited to individual needs
    • Assistance to maintain independence, safety, and wellbeing at home
    • Emotional and wellbeing support within the living environment
    • Support to develop daily living skills and routines
    • Collaboration with support networks and other service providers
    • Flexible supports that adapt as needs change
